Social Worker - Substance Use Disorder

The Substance Use Disorder (SUD) Intensive Outpatient Program (IOP) Social Worker provides therapeutic services to clients with substance use disorders and co-occurring mental health conditions. The Social Worker is responsible for conducting assessments, facilitating group and individual counseling, creating treatment plans, and ensuring compliance with regulatory standards. This role also includes collaboration with interdisciplinary teams to support clients in achieving their recovery goals.
RESPONSIBILITIES
Client Care:
- Conduct comprehensive biopsychosocial assessments and develop individualized treatment plans.
- Provide evidence-based individual, group, and family therapy to clients enrolled in the IOP.
- Collaborate with clients to identify recovery goals and track progress.
- Address co-occurring mental health conditions and provide crisis intervention as needed.
- Maintain client confidentiality in compliance with 42 C.F.R. Part 2 and HIPAA.
Program Support:
- Participate in multidisciplinary team meetings and case reviews.
- Facilitate psychoeducational workshops and therapeutic groups.
- Support program development by providing feedback on client needs and service improvements.
Documentation and Compliance:
- Maintain accurate and timely documentation in the electronic health record (EHR).
- Ensure all documentation meets organizational, state, and federal standards.
- Adhere to program policies, procedures, and ethical standards in all professional activities.
Collaboration and Advocacy:
- Serve as an advocate for clients, ensuring access to necessary resources and services.
- Build and maintain relationships with community organizations and referral sources.
- Provide education to clients and families about substance use disorders, treatment options, and recovery strategies.
